Pharmacology:

Nepafenac in FREDNEP-OD, when rapidly crosses the cornea, then gets deaminated by the internal Hydrolases into an activated form amide analogue, Amfenac, which posts inhibitory effect on the Cyclooxygenase derivatives COX1 & COX2.

As per the literary data, the COX-1 is associated with the inflammation whereas COX-2 is associated with pain. It works as per the NSAID Model, uniformly inhibiting the activity of COX1 and COX2 receptors, responsible for the cascade of inflammatory reactions. The duo plays vital role in the production of Prostaglandins and Thromboxane from Arachidonic acid through a series of reactions. The latter two are responsible for the series of actions take place in during the inflammation, like pain, itching, redness, discharges, etc.

The decrease in the concentrations of Postglandins also prevents the sensitization of the pain in the nerve receptors. Thus, relieving from the pain caused due to the inflammation.

It is dosed once rather than thrice daily, which improves outcomes of inflammatory markers in terms of lid edema, conjunctival congestion, and anterior chamber cells.

Indication and Usage:

Treating Inflammation And Pain Associated With Cataract Surgery, Acute Pseudophakic, Chronic Pseudophakic, And Recalcitrant Uveitic Cystoid Macular Edema (CME).

Dosage and Administration:

  • Instill One-Two Drop Once a day of FREDNEP-OD a day prior to the surgery.
  • Instill One-Two Drop Once a day of FREDNEP-OD post-surgery or as directed by the Physician

 Warnings & Precautions:

  • Prolonged use NSAIDS may cause Keratitis.
  • May slow or delay healing.
  • It should not be administered, wearing contact lenses.
  • In the case of corneal breakdown, discontinue the usage and contact your Eye Doctor.
  • Contraindicated is hypersensitive to the contents of the ophthalmic solution.

 Adverse Effect:

Decreased visual acuity, a feeling that something is in the eye, increase in IOP, sticky sensation, headache, ocular stinging or vision change.

Information to Patient:

Follow the instructions that are provided by your doctor and if the adverse effect persists, discontinue the usage and contact your doctor immediately.
